A Home Medicines Review is when an accredited consultant pharmacist reviews the medicines that you are taking while ensuring that you’re taking them correctly.
By offering at-home or in-residence visits, the pharmacist provides a comprehensive assessment that includes guidance for usage, timelines, side-effects, prescription medications, and patient questions.
How do I arrange for a HMR?
It’s easy.
It’s easy. First, speak with your doctor to ask for a referral. From there, the doctor will securely provide Medicosurveillance with information about your medical history. Lastly, we will reach out to you and set up a time to meet in the comfort of your home.
